The following describes our Mail-In and Hand-In Booth Ordering processes.

Mail-In Ordering

If you prefer to mail-in your order, print and fill out and sign the Booth Application Form. Keep the Terms of Service (ToS) pages for reference. Mail the application with payment to the address at the bottom of the Booth Application Form.  Include the $25 Application Processing Fee in your payment total.

Hand-In Ordering

If you prefer to hand-in your order, print and complete the Booth Application Form (keep the ToS pages for reference) to Forcht Bank at 102 N Public Square, Greensburg, KY 42743. Ask for Randy or leave your Booth Application Form and payment with a Forcht Bank employee. Include the $25 Application Processing Fee in your payment total.
